Senior Clinical Pharmacist – Rotational (Acute Hospital and GP Practice) 3 year Fixed Term

A collaborative bid to provide clinical pharmacy support in the general practice setting as part of NHS England’s second wave of their national programme has been successful in Lewisham. The integrated approach to medicines optimisation in Lewisham is now expanding with the General Practice Forward View.

An innovative and exciting opportunity has therefore arisen for a number of motivated and patient centred clinical pharmacists to work both within GP practices and within the acute hospital rotations at Queen Elizabeth Hospital.

Queen Elizabeth Hospital

The clinical pharmacy service to the wards is well established and provides high quality care across a variety of specialties. Joining a team of experienced pharmacists and technicians you will work as part of the multidisciplinary team to deliver a patient focused clinical pharmacy service to the wards. You will have the opportunity to rotate through a variety of specialities, including Medicine (Acute and Specialist), Elderly Care and Surgery.

GP Practice

The postholder will rotate to work as part of GP practice teams to provide specialist advice to patients, undertake medicines reconciliation and manage prescribing queries following discharge from hospital, provide leadership on person centred medicines optimisation and contribute to the quality and outcomes framework to support quality improvement in general practice.

Responsibilities include providing direct patient care and undertaking full, patient centred medication reviews to proactively manage people with complex polypharmacy particularly the elderly and those with multiple long term conditions in the general practice setting. The role with involve partnership working to co-ordinate medicines related aspects of care across health and social care interfaces.

Pharmacists will follow an education and training pathway as part of the General Practice Forward View programme.

The posts will be based at Queen Elizabeth Hospital, 3 year fixed term positions
