Specialist Services provides medium, low secure and step down in-patient services for over 150 service users in the Lancashire and South Cumbria area. The Community Business Unit comprises a variety of community based services (including the Forensic Outreach Service, forensic Community Mental Health Team, PD and ASC Gatekeeping, and Probation/Offender Pathways).

An exciting opportunity has arisen for a Team Practitioner to work within the Community Pathways, Forensic Outreach Service. Over the past twelve months Forensic Community Services have grown and the successful post holder will be part of a dynamic team. The Forensic Outreach Services main objective is to avoid admissions to Secure Services, by providing specialist risk assessment, formulation, risk management and consultation to local inpatient services and community services within the LCFT footprint of Lancashire and South Cumbria. We work with service users with severe and enduring mental health conditions including personality disorder.

The successful applicant will have the ability to work with service users, who present with a full range of clinical needs and risk behaviours, and the ability to engage with and respond to, highly complex clinical, professional and ethical challenges and demands on clinicians and services. The applicant will be involved in service development and assurance frameworks. The successful applicant will be open to new ways of working, foster and promote recovery based care, support and embrace proactively a drive to maintain Service Users in the least restrictive environment with safe and responsive packages of care.
